If you selected creditor attorney as the filer, a prompt will appear for you to add the name and address of the attorney. The list of creditors will now appear. The creditor's name and address must be an exact match. If a match is found, select the matching creditor. If not, create a new creditor record by selecting the radio button "creditor not listed". The fillable proof of claim form will now appear. The form displays the case number and debtor's name at the top. Verify this information.
